MyHealth Centre is now WELL Health Sleep Centre in Brampton! We're the same team with a new name, located in the Chrysler Drive Centre at the corner of Williams Parkway and Chrysler Drive. We provide OHIP-covered sleep consultations and sleep studies to treat chronic fatigue, excessive daytime sleepiness, insomnia, morning headaches, non-restorative sleep, restless legs, sleep apnea, and snoring. We are available to answer your phone calls and provide sleep consultations during our clinic hours of operation listed below.
We provide sleep studies during evenings, seven days a week, excluding holidays. We are not available to answer phone calls during evenings and weekends.

Step 1: Requisition
Download a requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.
Step 2: Schedule
All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.
Step 3: Prepare
Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***
*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, such as a family doctor, walk-in clinic or community health centre.
*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.
***If you have mobility concerns, please ask your caregiver to accompany and assist you.
We will send your diagnostic report to your referring healthcare provider, who will follow-up with you. We can send it to additional healthcare providers upon your request.
