3. Prepare
Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.**
*Our consultations and diagnostic tests require a signed requisition from a certified Ontario healthcare provider, such as a family doctor, walk-in-clinic or community health centre.
*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and Independent Health Facilities.
**A late arrival may require rescheduling your appointment. If you have mobility concerns, please ask your caregiver to accompany and assist you.
